On-Call Nurse
24 hours a day / 7 days a week

Full medical services of traditional home health for treatments rendered by RNs, and LVNs consistent with a treatment plan established by the patient's attending physician.

Care at home is medically necessary for the patient
Patient is under a physician order plan of care
Patient requires one of the following services: intermittent skilled nursing, continued occupational therapy, physical therapy, speech therapy or medication management
Patient requires assistance to leave home or is homebound
Services are provided to the patient by a Medicare certified agency like N Your Home Health Care
